Cholangioscopy - Directed Radiofrequency Ablation of Complex Biliary Cholangiocarcinoma by Dr Nishmi Gunasingam and Prof. Philip I Craig, Department of Gastroenterology and Hepatology, St George Hospital, Sydney, NSW, Australia

A 64-year-old patient has been referred for cholangiocarcinoma. Previous chimiotherapy and ERCP procedures have failed.
Dr. Gunasingam performed an RFA procedure using the Habib™ EndoHPB RF Catheter.
In this case report, he will show the impact of RFA on complex biliary cholangiocarcinoma:

• Pre-RFA: Cholangioscopic imaging showing hilar obstruction.
• Post-RFA: Cholangioscopic imaging showing changes immediately after radiofrequency ablation.
• 6 weeks post RFA: Cholangioscopic imaging showing changes 6 weeks after radiofrequency ablation.

The patient was a 75 year old woman diagnosed with cholangiocarcinoma in the CBD 1-1/2 years prior. The patient had undergone multiple plastic stent placements and exchanges, and subsequent placement of an uncovered WallFlex™ Biliary RX Stent (10x60mm) at another medical facility. The patient was receiving chemotherapy. In a repeat ERCP with cholangioscopy a tumor ingrowth of the stent was revealed and confirmed by an intraductal biopsy. The treating physician referred the patient for an RFA procedure.
