Indigent Care and Humanitarian Product Donations
We may support product donations in support of humanitarian missions and indigent patient care product donations for select products.

We receive many worthwhile requests. Unfortunately, due to budget limitations, we are unable to fund them all. It is important to note that past funding does not guarantee future approval and that submissions can be approved at an amount less than the requested amount. Do not assume that a request has been approved. Requestors will receive an email notification of approval or denial after the Boston Scientific committee has made its decisions.
The U.S. Physician Payment Sunshine Act requires all pharmaceutical, biologic and medical device manufacturers – like Boston Scientific – to report annually to the U.S. government payments and transfers of value made to U.S. physicians and teaching hospitals. This could include educational and research grants and charitable donations.

Indigent Patient Grant Requirements
We may provide a product to an institution where a patient is uninsured or underinsured, and is unable to cover the copayment through an Indigent Grant.
- A completed Indigent Patient Grant Application submitted within 30 days of surgery using our online tool. For Watchman requests, application must be submitted prior to any procedure date.
- A letter of request on the institution’s letterhead that includes:
- Patient name (required for FDA purposes)
- Patient need
- Procedure(s) expected to be performed
- Product name, size and quantity
- Date / Anticipated Date of procedure
- Confirmation that neither the institution nor the physician will bill for patient services
- Completed W-9 form (W-9 forms can be found on the IRS website.)
Humanitarian Mission Grant Requirements
We may provide a product to a Health Care Provider (HCP) where the HCP is providing care to indigent patients outside of his/her geographical area through a Humanitarian Grant. An HCP from "Doctors without Borders/ Médecins Sans Frontières" could request this type of grant.
- A completed Humanitarian Mission Grant Application using our online tool
- A letter of request on the institution’s letterhead that includes:
- Community to be served
- Number of patients expected to be treated
- Procedure(s) expected to be performed
- Number of Health Care Providers participating
- Product name, size and quantity
- December 2014 IRS issued W-9 form (W-9 forms can be found on the IRS website.)
