Obsidio™ Conformable Embolic

Pre-operative right humerus lesion embolization

Presentation

72-year-old man with history of multiple myeloma presented with a right humerus lytic lesion and limited range of motion. Interventional Radiology was consulted for pre-operative embolization prior to surgical fixation.

Intervention used

Right radial access was gained and a 5 F sheath was placed. A 5 F angled catheter was placed in the right subclavian artery and an angiogram was performed. Angiography revealed a hypervascular lesion in the right humerus supplied by multiple vessels. A 2.8 F microcatheter and 0.014” wire were used to select the first vessel supplying the lesion and embolization performed with a 0.3 cc aliquot of Obsidio Embolic. The microcatheter was removed, flushed and used again to select two additional branches where 0.2 cc of Obsidio Embolic was deployed into each vessel. A total of 3 vessels were embolized with a total of approximately 0.7 cc of Obsidio Embolic. Obsidio Embolic casts in the vessels is highlighted in post-imaging. Completion angiography via the base catheter showed complete embolization of the left internal mammary artery. A still image shows Obsidio Embolic cast occluding the vessel.

Outcome

The patient underwent successful surgical fixation with minimal blood loss.
